A Fistula-in-Ano is a track lined by granulation tissue which connects the anal canal mucosa with the skin around the anus. The fistula proceeds to discharge and the continual reinfection from the anal canal prevent healing of the Fistula.

The incidence of Ano Rectal diseases is increasing day by day due to fast food culture, Low fibre diet, sedentary lifestyle etc.

Causes:

  • The anal fissure normally occurs when the anal tissue is damaged during a hard and dry bowel movement which tears the anal lining,
  • The anal fissure can also increase due to higher than normal pressure in the anal sphincters. Diarrhoea and inflammation of the anorectic area,
  • Many women during childbirth develop an anal fissure,
  • Other causes of the anal fissure are digital insertion (during examination), foreign body insertion, or anal intercourse.

In some cases, the anal fissure may be effected by other health provisions, such as: vitaminB6 deficiency, abdominal pain, fever, weight loss, crohn’s disease, inflammatory bowel disease (IBD) that causes bloody diarrhoea, syphilis, a suppressed immune system, tuberculosis, HIV infection, anal cancer; a low fibre diet may also contribute to the development of a fissure.

Identification of Fistulous Tract is by:

Physical examination, Digital Rectal Examination, Proctoscopy, Probe test, Injection technique, Fistulography, Colonoscopy, Sigmoidoscopy, Barium enema, and Anorectal manometry etc.
